504 Education Plan

If a student has a health issue that impacts them academically but they do not qualify for an IEP, a 504 plan may be useful.

 

Current and robust medical information must be provided to justify a 504 plan and parents must be in agreement. Some examples of situations that justify a 504 plan include but are not limited to:

  • Diabetes that requires a health paraprofessional.

  • Medically documented ADHD that may necessitate testing accommodations.

  • A broken arm that requires a temporary scribe. 

Please see Sarah Stevens in 113 to discuss this process if you have a student that qualifies. The process will be facilitated by the 504 coordinator.
